PRIMARY WORKER'S COMPENSATION INSURANCE PROGRAM

An innovative, fully insured group dividend plan whose goal is to reduce members' costs for Worker's Compensation insurance is now available as part of this program. This "state-of-the-art" CVMIC Worker's Compensation Program is designed to promote conscious work-safety practices that will result in fewer employee injuries and illnesses.

The program provides all coverage and limits required by the State of Wisconsin, insures all participating members from the "ground up" (i.e., no deductibles), and has the potential to pay larger dividends for favorable claims experience than most other programs of this type.

Highlights of the program are as follows:

Superior Dividend Structure: Designed to result in the lowest net cost for your community.
"A (Exellent) Rated Reinsurer (Best's Key Rating Guide): Underwritten by Safety National Casualty Corporation."
No Assessment Liability: Members will not have to repay any dividends paid to them nor will insurance costs exceed the standard premium for Worker's compensation, i.e., there is no retro penalty.
Automatic Level Dividend: Returns 10% of the earned premium to members, irrespective of loss experience.
Additional Safety Dividend: Based upon the group's actual developed losses, the plan could return up to 70% of member premiums.
Large Loss Protection: Member cities will not be unfairly penalized for a catastrophic claim which could otherwise eliminate all earned dividends.
CVMIC-Employed Loss Control Specialist: CVMIC staff includes three full time loss control specialists to directly service all member communities
Dedicated Claims Services Representative: A Wisconsin-based Senior Claims Representative manages all Worker's Compensation claims for CVMIC.

WORKER'S COMPENSATION CLAIMS REPORTING PROCEDURES

In cases of serious injury or death, report the claim immediately to CVMIC via telephone and send in written notice of claim within 24 hours.
For all other claims, send in written notice of claim within 7 calendar days of the date of loss.
Keep a written record of all injuries even if no medical treatment is sought. If no medical treatment is rendered and no time is lost from work, reporting the claim to CVMIC is not required but can be done at the employer's discretion.
In case of emergency, the employer has the right to direct the injured worker to a medical provider of the employer's choice. Once the emergency has passed, the employee has the right to choose his or her own doctor as well as the right to get a second opinion.
